While bigger restaurants in Kuta accept cards, local warungs, surf instructors, and beach parking attendants only take cash. ATMs can run out of money on weekends, so stock up.

Using VISA credit cards we paid 0 ATM fee when BRI ATMs - this is probably the cheapest option depending on your banks fees for currency conversion.

Always (!) chose your card to be charged in Indonesian Rupiah when withdrawing - the proposed rate (if you let the ATM provider do the exchange) are horrible.

If you want to make use of currency changers be careful and count your money when receiving it.
